Chronic Disease Management: In Practice

Students must be working in a clinical area that provides access to Primary Care type patients with chronic conditions. Access to patient data for clinical audit purpose is required.

This subject focuses on the role of the primary care nurse in providing patient centred care within the context of chronic disease management. The lived experience of patients with chronic conditions is explored alongside the scope of nurse practice in the provision of clinical care, psychosocial support and patient education within a multidisciplinary framework. This subject builds on the knowledge of the pre-requisite subject, NURS90104 Chronic Disease Management: Foundations, and assists nurses in promoting chronic disease self-management based on the chronic care model.

At the completion of the subject students should be able to:

• Complete patient centred chronic disease management plans using current evidence based guidelines
• Understand the importance of self-management in chronic disease and the barriers patients may face in adhering to recommendations
• Contribute to a multidisciplinary team approach to care for individuals with common chronic diseases
• Establish and maintain good linkages with community resources and services
• Provide effective education and support in the management of common chronic diseases using an empathetic and non–judgemental approach

• Case study equivalent to 2000 words, Mid Semester, 30%
• Resource Portfolio equivalent to 2500 words, End Semester, 60%
• Clinical Audit equivalent to 500 words, End Semester, 10% (Hurdle Requirement)

In order to pass this hurdle requirement, students must demonstrate that they have analysed the data, compared the results to the recommended clinical targets and evaluated the barriers/enablers that may account for discrepancies.

• Reflective journal 1000 words, End Semester, Pass/Fail (Hurdle Requirement)

Students must achieve a cumulative mark of at least 50% and a pass in the hurdle requirement in order to pass the subject.
