This subject focuses on the role of the primary care nurse in providing patient centred care to individuals with diabetes, asthma, chronic obstructive airways disease, hypertension and heart failure. The course content includes the pathophysiology of these common conditions and the common pharmacological agents used in treatment. The Primary Care system in Australia is explained and the systems and evidence based management principles explored with a focus on working within a multidisciplinary framework.

At the completion of the subject students should be able to:

• Understand the pathophysiology of diabetes, asthma, chronic obstructive airways disease, hypertension and heart failure
• Describe the actions of some common pharmaceutical agents used in the treatment of these conditions
• Describe the current management guidelines for these conditions
• Appreciate the importance of patient centred care in the management of chronic disease
• Develop an appreciation of the social determinants of health on the impact of chronic disease

• Written Assessment equivalent to 1000 words, Mid Semester, 20%
• Multiple choice/short answer tests on LMS equivalent to 1000 words, End Semester, 20%
• Written Assignment equivalent to 2000 words, End Semester, 60%
• Reflective journal equivalent to 1000 words, End Semester, Pass/Fail (Hurdle Requirement)

In order to pass this hurdle requirement, students must demonstrate that they have read and understood the materials related to the tasks and can apply the principals and evidence based guidelines to the context in which they work.

Students must achieve a cumulative mark of at least 50% and a pass in the hurdle requirement in order to pass the subject.

This subject is not available as a breadth subject.
